ITS offers two options for creating web forms.   These options are suitable for collecting certain types of data via informal surveys not subject to IRB review. Sensitive and confidential data always requires special care. Such information may be collected and stored, when appropriate, in UAlbany-hosted services (such as Wikis @ UAlbany) or UAlbany-approved cloud services (including Microsoft Forms).

Microsoft Forms:

  • With Microsoft Forms, you can create surveys, quizzes, and polls, and easily see results as they come in.  
  • You can add a type of question to your form that allows for file uploads.
  • Forms can be created and owned by an individual or by an Office 365 Group or Microsoft Team.  
    • If an individual creates and owns a form, the data submitted for that form will be stored in that person's OneDrive for Business space.
    • If the data being collected needs to be shared by a group, the Form should be created as a Group Form, and the data submitted for that form will be stored in the Office 365 Group or Microsoft Team where it was created.

Wiki Forms:

  • Forms for Confluence is a simple and effective tool for creating forms (surveys, quizzes, and polls) directly in your Confluence wiki pages.
  • You can add a type of question to your form that allows for file uploads.
  • Responses to a form can be either stored in the internal database in the wiki or sent to a specific email address - or both.  The most common use is to email the results.
