The Faculty; Educational Offerings and General Education; Leadership, Governance, and Administration; and Student Support Services subcommittees commissioned this survey during December 2008 to address specific questions that they needed additional information on in order to answer their charge questions. Therefore, the survey results should not be interpreted as summative evaluations, in and of themselves, of particular issues, programs, or offices.

The survey results are being publically shared, at the behest of the self-study Subcommittees, in the belief that the benefits of releasing the information gathered exceed any risks that invividuals or other parties will present information out of context.

Survey Instruments

Student Survey - PDF of the survey instrument, skip logic used to present different questions to different groups of students (e.g., special questions for transfer or non-traditional students)

Faculty and Staff Survey - PDF of the survey instrument, skip logic used to present different questions to different groups (e.g., some questions only for teaching faculty, professional staff not given teaching questions, but are given governance questions, PT faculty only get the teaching questions, not the governance questions, etc.)

Note:  The PDF files "approximate" but are not exact graphical duplicates of the actual survey instruments.  These PDF files intended to make known to members of the various subcommittes what questions were asked in the event the response data can can also be of use to their work.
